We ask registrants to identify their age category to ensure they are 18+ and eligible for membership, to help us understand our Elevate membership and ensure it is representative of our wider Canadian CF community: 

Connection to CF (Select all that apply)  

We ask registrants to identify their connection to cystic fibrosis to ensure that program input reflects the lived experiences of people living with or impacted by CF. 

Gender (Optional) 

By choosing to disclose gender identity, registrants can help us better understand our Elevate membership and ensure it is representative of our wider Canadian CF community.
